摘要 烧伤伤口在临床上带来了重大挑战,如出血、感染、过度炎症、瘢痕形成以及敷料管理困难。现有敷料往往缺乏多功能性,无法提供平衡的粘附力和可控的剥离性。为解决这些问题,我们开发了一种具有可逆粘附性的温度响应型水凝胶粘附敷料,该敷料具有止血、抗菌、抗炎和防瘢痕功能。该水凝胶粘附敷料可通过简单的两步混合工艺轻松制备,该工艺涉及用鞣酸(TA)和甲酰基苯基硼酸(FPBA)桥接明胶和季铵化壳聚糖(QCS),通过硼酸酯键和静电相互作用形成双动态交联网络。得益于其双网络结构设计,该粘附敷料表现出强大的组织粘附力(481.13 J m-2),并在大鼠尾部出血模型中实现了20秒内快速止血,与商用止血剂相比,失血量减少了78.26%。此外,冰敷1分钟即可使界面韧性迅速降低至166.53 J m-2,仅为商用水凝胶敷料的40.51%,从而实现无痛移除。此外,该粘附敷料还表现出广谱抗菌活性、强抗氧化作用和活性氧(ROS)响应性药物释放。这些特性共同将二度感染烧伤的愈合时间缩短至21天,并与商用磺胺嘧啶银(SSD)敷料相比,提供了更优的无瘢痕修复效果。本研究提供了一种简单而有效的策略,用于设计兼具强粘附力和可控剥离性的多功能敷料,为感染性烧伤伤口的全周期管理提供了新颖的解决方案。
Abstract
Burn wounds pose significant clinical challenges such as hemorrhage, infection, excessive inflammation, scarring, and difficulties in dressing management. Existing dressings often lack multifunctionality and fail to provide balanced adhesion and controllable detachment. To address these issues, we developed a temperature-responsive hydrogel adhesive dressing with reversible adhesion that offers hemostatic, antimicrobial, anti-inflammatory, and scar-preventing functions. The hydrogel adhesive dressing was readily fabricated via a straightforward two-step blending process, which involved bridging gelatin and quaternized chitosan (QCS) with tannic acid (TA) and formylphenylboronic acid (FPBA) to form dual dynamic crosslinking network through borate ester bonds and electrostatic interactions. Benefiting from its dual-network structural design, the adhesive dressing exhibited strong tissue adhesion (481.13 J m-2) and achieved rapid hemostasis within 20 s in the rat tail bleeding model, reducing blood loss by 78.26 % compared to commercial hemostatic agents. Moreover, one minute of ice pack treatment rapidly reduced the interfacial toughness to 166.53 J m-2, which is just 40.51 % of that of commercial hydrocolloid dressings, enabling painless removal. Furthermore, the adhesive dressing demonstrated broad-spectrum antimicrobial activity, potent antioxidant effects, and reactive oxygen species (ROS)-responsive drug release. These attributes collectively reduced healing time to 21 days in second-degree infected burns and provided superior scarless repair compared to commercial silver sulfadiazine (SSD) dressings. This study provides a simple yet effective strategy for designing multifunctional dressings that combine robust adhesion with controllable detachment, offering a novel solution for the full-cycle management of infected burn wounds.
